With the marked increase in life expectancy achieved during the past century, we can expect that the elderly population will continue to grow disproportionately during the next 50 years. The oldest of the elderly, persons 85 years of age or older, now represent the fastest-growing group not only in developed countries as a whole, but also in emergency departments, intensive care units, and hospital wards. In recent years, much has been learned about the physiologic and pathologic consequences of aging and the varied clinical manifestations of disease in the elderly. The spectrum of illness in middle-aged adults is similar to that in older adults, but the prevalence of various entities differs. In elderly people with diminished functional reserve and coexisting illness, an acute disease often causes perturbations in multiple systems. Clinical presentations may be relatively nonspecific, and mortality is almost always higher than in younger patients. Consequently, diagnostic and therapeutic planning for older patients must be tailored to meet their differing needs and objectives. Unfortunately, there are comparatively few off-the-shelf works a busy clinician can refer to when caring for acutely ill elderly patients. Acute Emergencies and Critical Care of the Geriatric Patient will help to fill this void.

This book consists of four sections that cover the basic principles of critical care, topics related to surgical emergencies and medical emergencies, and special issues. A strength of the book is that each of its 25 chapters can serve as a stand-alone reference for rapid review. Yet the text flows nicely from chapter to chapter, making it easy to garner a comprehensive survey of this area of medicine. The chapters are consistently organized, with clear headings in outline style. As a multiauthored book, it benefits from strict adherence to this unified organization. There are many easy-to-read summary tablews as well as numerous figures that enhance the text. The chapters are well referenced, are organized according to system, and provide useful, though somewhat broad, surveys. Throughout, several thematic issues -- including reduced functional reserve in various organ systems (homeostenosis) and atypical or muted clinical presentations -- and an emphasis on general principles combine to form a cohesive and useful work.

The first section of the book, "Principles of Geriatric Critical Care," focuses on older patients in the emergency department and critical care unit, with particular attention to geriatric pharmacology and the unique nursing needs of the elderly. The second section, "Surgical Emergencies," provides a review of preoperative evaluation, emergency anesthesia, common injuries, the acute abdomen, and vascular catastrophes. Although it is solid and filled with interesting teaching pearls, this section suffers somewhat in comparison with the other sections, especially because of a tendency toward oversimplification of complex concepts.

The chapters in the section entitled "Medical Emergencies" are very well written and provide an outstanding survey of internal medicine in the acute care of elderly patients. Although the chapters overlap somewhat, they are generally complementary and reinforce important facts and concepts. The final section in the book, "Special Issues of Aging," deals with a variety of other important topics related to geriatric acute care, such as elder abuse and acute loss of vision.

This book would be a very good supplement to a complete textbook of medicine. Consistently emphasizing relevant general principles, it will enhance readers' understanding of many problems in the acute care of elderly patients. It will be an excellent resource for all those who care for the elderly in acute care settings.

Second to none

A few years ago I read a short book on the Pacific War written by H.P. Wilmott. For those who don't know of him, Wilmott taught at Sandhurst for many years, and is probably one of the top two or three historians of the Pacific War. At the end of the book there was an annotated bibliography. I always like this, as opposed to a regular bibliography, because the annotations tell you what the author thinks of various sources, and that in turn tells you something about his thinking. But sometimes the recommendation is more direct: you trust the author, and he tells you something is worthwhile. In this instance, he said that he wasn't sure how Pacific War historians got along before the book Kaigun was published. Needless to say I went out and got one. It's not a cheap book, there aren't used copies cheaper, there's no paperback available. It's still worth every penny, and I will tell you below why you need it.

There are, of course, a bushel and a half of books about the war in the Pacific during World War II. Many of them note that the Japanese fought the war in an unusual fashion, and most note differences in technology, strategy, tactics, and philosophy. Some of these things are vaguely explained by the differences in Western and Japanese society, but at best the explanations are vague. This leaves a huge gap in the history of the Pacific War.

Kaigun fills this gap. The authors basically explain every question of this type involving the Japanese Navy in World War II, from why their cruisers had long ranged torpedoes to why their navy's intelligence was so poor to why they insisted in planning as if the American Navy would act in particular ways (even after it had demonstrated that it would act in other ways). Much of the differences between the Japanese and American Navies stemmed from the fact that the Japanese had to modernize so quickly. This circumstance had background in that Japan had little naval tradition, and no ships really on which to base a modern navy. While this meant that Japanese Naval modernization had many pitfalls, it also had advantages, notably a lack of old traditions and hoary rivalries that plagued other nations' Naval modernization programs.

The authors spend a considerable length of time discussing Japan's wars with first China and then Russia in the early stages of their modernization. China proved little challenge in a Naval sense, but Russia was a much more formidable opponent, though the Japanese defeated them decisively. While the war itself is discussed at some length, a great deal of time is also spent discussing the aftermath, and the war's influence on Japan's strategy, tactics, and philosophy in future wars. As far as the authors are concerned, Tsushima was the principle reason the Japanese Navy spent most of the thirties planning for a major confrontation between the U.S. Navy and their own combined fleet. The latter confrontation was apparently modeled rather closely on Tsushima, which was pretty much the same strategically, though the directions and the scale were of course different.

This book is full of information, much of it not available elsewhere in English. Frankly, if you are interested in the War in the Pacific, I agree with Wilmott, adding my faint echo to his endorsement: this book is invaluable. It's also, as a side note, very well written. Highly recommended.

Important new work on Japanese naval doctrine and tactics

I did some of the illustrations for the book, so maybe I am biased, but this book will be read by people interested in this area of naval history for years to come. David Evans and Mark Peattie have researched this work meticulously, (including using previously untranslated Japanese primary sources from the Japanese Self-Defense Force archives) to construct a view of the changing inter-workings of technology, strategy, and tactics in the Imperial Japanese Navy. The writing is excellent, the layout is elegant, and the maps and illustrations (ahem) are superb. The book also contains excellent back-matter in the endnotes, bibliography, and index. A MUST for any serious student of the Imperial Japanese Navy.

Fascinating

Kaigun Strategy is a book that I had a lot of trouble putting down. This is perhaps strange for a book which discusses things such as the development of dry steam as a more efficient mode of propulsion, change in the composition of steel as it related to battleship construction and other very technical subjects. In a way the book said a lot about the Pacific War that had been said before. Yet in a way it was as startling revelation of the development of Japanese Society from the Meiji Restoration to the end of the second World War. The book is a discussion about the development of Japans Navy. Until 1945 the Japanese spent the staggering amount of 27% of their national budget on the navy. The reason for this lies with the now forgotten writings of Admiral Mahon.
Mahon was an American who in his day was as famous as Marx and Lenin. He wrote a history of sea power a book that argued that the reason for the wealth of great nations was the control of an empire through control of the sea. The Japanese were converts to his doctrines and being an island power thought that the key to the national destiny was the acquisition of empire. Kaigun Strategy is a study of how the Japanese Navy tried to develop a navy that would give them preponderance over that of the much stronger American Navy. The book goes into great detail about how the Japanese studied the most modern technology to develop a numerically inferior but well trained modern Navy. The belief in empire and the need to ensure oil supplies put Japan on a collision course with the United States of America.
The end of the war has led to Japan sheathing the sword and seeking to build up a strong economy. This has led to Japan becoming one of the richest and strongest countries in the world. How more productive that has been rather than putting most of the national wealth into a Navy which ended up on the bottom of the sea.
The book is fascinating at showing that whilst a large amount of Japans planning and development showed tremendous skill and intellect, at the same time ridiculous errors were made. Thus whilst Japan build up a modern fleet and air wing it failed to: · Adopt a convoy system during the war or to arm enough destroyers with sonar equipment to protect its merchant marine. · Did not realise till after the war started that there were not enough tankers in the possession of Japan to move enough oil from its new possessions to keep both the navy and industry going. · Made no attempts to develop code breaking in the way that its Axis Partner Germany and the Allies did. · Were not able to adopt the strategy once it became obvious that the war was evolving into one of attrition rather than a single decisive battle.
The book is a fascinating one and shows how the history of nations can be molded by the history of ideas.

This explains both the development of a Kashmiri Muslim consciousness and the emergence of a political movement which remains in thrall to a religiosity thrust upon it for the past 150 years. If you like to read books about funny girls, friendship, falling in like, good friends, being your self, oh and did I mention friendship and good friends ???? A Classic Jill Krementz Book

While Jill Krementz is a talented photographer, what my daughter and I really loved about this series was the very detailed text. She was fascinated to learn that each level at the School of American Ballet wears a different color leotard. She sympathized with the dancer who wants a new role in The Nutcracker, but gets the same casting. She learned the difference between a class, a rehearsal, and a performance - all opportunities to dance, but each with a specific purpose.
The original Jill Krementz books, like this one, were printed in black and white. A few later books ("A Very Young Skier") are in full color. But while the color photos look more contemporary and appealing, the later books are "dumbed down" - the text is very short, more like picture captions than narrative.
For the wonderful sense of detail you need the classic titles like "A Very Young Dancer." They make a dancer's life so real, a young reader can easily picture herself (or himself) doing the same. And that's the real power of this series: instead of being a passive spectator, the young reader becomes a participant. My daughter is now dancing in The Nutcracker for her ninth (and last) year. I think "A Very Young Dancer" was a big help along the way.

A ballet dancer's review

I am a ballet dancer. I have been dancing for 15 years now, and this is still one of my favorite books. I was introduced to it when I was about 5 and it facinated me. The whole story seemed true to what I knew about ballet and Stephanie, the main character, lived my biggest dreams! For years, I only borrowed it from the library, and soon it was one of the most worn in their collection! Finally, we moved away and the next library didn't have it, but Mom found it at a used book store and bought it for me. I ended up meeting David Richards, a teacher in the book, at a ballet competition and he signed it for me. He told me that Stephanie doesn't dance any more, and I don't think she ever made it into New York City Ballet company or any other ballet company. Like I said earlier, I've been dancing 15 years and this book, to me, is even more a true embodiment of what the every-day life of a ballet dancer is! I've known this book so long that I can quote it forwards and backwards, and almest every sentence is or has been true for me! I just finished a run of Nutcracker and, "I tore a little pink ribbon off the pantaloons that I wore in the party scene to keep for a souvenir."-to quote page 118. I am positive that current dancers, former dancers, future dancers, and sancers in their dreams will absolutly love this book, especially because it is so true to what they feel!

AN INSTANT CLASSIC!, It Should Be Reprinted...

I received this book as a Christmas gift around 1980, I believe. I cannot tell you how much this book meant to me as a girl. I must have read it about 200 times. My mother recently moved and was going through all our childhood things and found the book packed away. She gave it to me. I was so delighted because I thought I'd lost the book years ago. I sat down immediately and read it again. And yep...its still as good as it ever was.

I was one of the guides on this trip and really appreciate the sensitive and detailed story of the trip and how the book weaves in the much larger issues of climate change. The trip was sponsored by the Sierra Club which has been one of the leading voices in climate change. This is an issue lthat we cannot turn our backs from. While our story is a dramatic example, reducing carbon emissions through eliminating coal plants, reducing our dependence on automobiles, developing smart growth cities and other major changes are most important. I hope this well written story moves us all a bit.

Captured well in the title, this book tells parallel stories that exemplify our interconnection and impact. For the hikers, their families and loved ones, spread out and tucked in places far and wide, the terror at the top of the world can be replaced with relief and gratitude. The parallel story, told deftly by Ms. Shankman, has continuous, ongoing impact with profound consequences for all of us, and is an undeniable one. I appreciate the hikers, their willingness to share and the author for her work.

If you are a student of God's Word, then you need to purchase this book. It is an easy book to read, and one that every student needs. It has topics like Atonement, Faith,Dispensation and Ages, the Antichrist, The Church, the Jews. It is filled with information on these topics, and is filled with scripture. If you like Dispensationism, or if you aren't a follower of this type of study, you will learn a lot from Clarence Larkin's "Rightly Dividing the Truth".

This is an incredible little book packed with Biblical helps. Great diagrams regarding prophecy. Clearly and accurately deals with many subjects from a literal and dispensational view of interpreting the scriptures. I have had and used this book as a pastor for many years (it was a gift to me 25 years ago); this recent purchase was for a young believer in Christ who read it through and loved it.

A must read

This book is a must read for all Christians. It is so informative and will help people to understand the Bible better. The truths in this book are not being taught in our pulpits today nor are they taught in the Sunday Schools.
Christians must wake up and start rightly dividing the Word of God and then they will have a better understanding of the Scriptures and Clarence Larkin has done that with this book.

Professor Ake draws upon his expertise as a professional pianist and composer to present an impressively "reader friendly" historical survey of how jazz musicians and their audiences evolved an understanding and appreciation for this unique music which reflected the roots of the Black American subculture, went on to cross over into the dominant white popular culture, and go on to eventually achieve a world wide acclaim. Of special interest are Ake's commentaries on the concept of a "jazz standard" in the decade of the 1990s and how jazz history has never been a linear evolvement of musical styles, but a potpourri of disparate (and sometimes conflicting) creations, compositions, attitudes, performers and performances. Wide but not Deep

I don't know, Leonard, I think I'm going to have to give it three - well, it's not quite four. Maybe three and a half. It sounds like some of that stuff that's coming from some of those New Academic bands - where's that - Guelph? Also at Columbia there's a group of those guys. George Lewis can play that stuff, but some of the guys that follow George, well, they just haven't paid their dues! You can't just LEAP into the Post - Modern. You gotta know the tradition. It's not the quality of the questions. The questions are great. The answers? The answers are just too basic and self - satisfied and lacking in nuance. But you know, it's like Mintons and Joe Guy. A lot of the older cats, they learned about what Bebop was gonna be as a trumpet language from Joe Guy. And then Dizzy and Fats came along and washed him away. It's like, if you were gonna provide a real alternative to Roy Eldridge, you'd have to be able to have assimilated Roy in order to provide a really effective next step. These guys couldn't get on the bandstand in Paris w/ Barthes or Jacques D. (zikhrono livrakha) in a cutting contest. They'd get blown away! I mean it's cool that American Jazz Cats are trying to speak this language. But France is where the real stuff is. A guy who's trying to learn this stuff just from books and who hasn't really done their time defending it in the cafés - well, maybe in a generation or two, some little guy from Lawrence, Kansas or Grinnell, Iowa might be able to really swing and play this kind of blues. It's not ethnicity - it's GROWING Up with this kind of language as your culture! I don't think I like this one as much as that Robert Walser one you played before.
